POWERS Participant Portal Registration
The Participant Portal Registration form is intended for those individuals who have been enrolled into the POWERS study.
At the time of enrollment into the POWERS study the study staff will provide you with a User Name that is required to
register for the Participant Portal. If you did not receive your User Name, please 'Quit' and contact the POWERS
research staff to confirm your enrollment and obtain your User Name.
If you believe you have been assigned a Participant Portal account but you do not know your password and/or user name,
please ‘Quit' and use the 'Forgot your username or password?' option that is available on the Login page.
To request access to the POWERS Study Participant Portal, please fill out the form below and we will send email
confirming your registration.
